What do you do after you turn 50 years old? At
Androscoggin Home Care & Hospice, we prepare for another 50 years. Yes, our agency will be turning 50 years old in January of 2016 and will be celebrating the 10 year anniversary of our Hospice House this year in November. We have created a committee made up of volunteers and staff members that began meeting earlier this year to develop a celebration plan for these two milestones. Our goals are to celebrate the past, share with the community what services we have to offer today, and to explore the future opportunities that will help us continue to provide quality care and comfort to the families and patients that we care for.
Looking back, our Board of Directors developed and implemented a Strategic Plan committing our Agency to concentrate on areas of: Quality, with the goal of improving quality indicators for home care and hospice, as well as patient satisfaction to rise above national benchmarks; Finances, ensuring that our agency is financially responsible and developing a philanthropy plan that will allow our agency to move forward in the years to come; Marketing, strengthening our agency brand and increasing visibility of the services we offer; Leadership and Staff Development, ensuring that we have a knowledgeable staff that can lead us into the future; Expanding our Program Initiatives, creating a focus of strategic participation with area hospitals, physicians, and payors as well as looking at new program opportunities such as telehealth, palliative care, community care and chronic care programs to provide comprehensive services for our patients and their families.
Because of this strategic plan, there is a sense of trust and connection. AHCH staff, families, partner agencies, and caregivers feel engaged and are coming together to create an inclusive and healthy community. We can accomplish so much more together than we can individually. As an agency, we are working hard at forming partnerships, expanding relationships, and implementing new programs that will help us navigate the challenges that we are all facing with the new regulatory changes from the Centers of Medicare and Medicaid Services. AHCH strives to benefit the residents of the communities we serve through skilled home care, hospice care, and supportive care programs, while supporting the communities in which we live and work. As part of that commitment, AHCH provides high quality care to anyone in need who qualifies for services regardless of their ability to pay. Above all, AHCH’s guiding philosophy is that the needs of the patient come first. Our agency has a long-standing commitment to provide care to all those who require skilled or hospice services and that is why our Annual Fund

Mission of Service
• 6,184 skilled home care patients benefited from services that enable them to recover from illness or injury safely and comfortably at home. • 1,310 hospice patients were comforted and cared for in their final days. • 712 patients who lacked health insurance coverage received care in our home care and hospice programs, representing an investment of $1,131,084. • 140 individuals are able to live at home safely and with added confidence thanks to AHCH’s Supportive Care and Lifeline services. • 2,158 patients were managed through our Community Care Team to support them in improving their health goals and achieving better health outcomes. • 555 maternal and child home visits were made to families through our State funded Family Health Promotion Grant. • 168 patients and their physicians were assisted in evaluation, assessment and care coordination needs through our Compass Care Consultation Program.

My Grandmother didn’t want anyone to go into her house and take care of her husband. Then we meet Patti. Such a wonderful lady. She brightened up the room the minute she walked in. She never lied or sugar coated anything, no matter what the answer was. She had compassion and truly cared for the one man I called Grampa. Her helper, Katie, was a blessing! She explained everything to my Gram: what she was going to do for my Grandfather, step by step. She asked my Grandmother if she had any questions or

Giving to Androscoggin Home Care & Hospice is more than just the act of writing a check to our Annual Fund, attending an event, making a planned gift or making a gift in memory of a loved one. It is about people and the relationships that are forged between the patient, their families and the caregiver. Through their experiences, gratitude and stories we come to learn why they feel our Agency is so important to support. “Six months ago my life hit a bump, and I learned so much more about AHCH and the level of care, support, comfort and friendship that they provide to their patients and family members. In January of 2015 my brother, Tom, who was battling a mental illness, was diagnosed with stage 4 lung cancer. Tom had been living with my family since my Mom passed away, we were lost. I am not sure how we would have managed my brother’s care without the support and care from AHCH. They were there for us seven days a week, 24 hours a day – if we needed to reorder a medication, supplies, needed respite, volunteer support, or just someone to talk with. They were the first ones to call us when Tom passed away to give us support and remind us about the resources that are available to us. Our care team was always there for us and still are, for that alone we are grateful. The quality of care and kindness my brother and my family received was beyond anything I could have expected. I always knew that the care and support AHCH provided was outstanding, but until you actually need their services, you may not understand how special this agency is or the compassion their employees have in caring for a loved one.” ~ Jim Bouchard I hope you will join me in honoring the people listed in this publication through whose generosity supported our agency in 2014 by donating over $576,514. 2014 Hospice House Fund Memorial Gifts The Hospice House Fund was established in 2006 to benefit this special facility and the patients and families it serves.
Through this fund –
• Patients needing specialized in-patient level hospice care are served regardless of health insurance coverage. • Families with loved ones at the Hospice House are comforted and supported with onsite services and amenities. • Hospice House staff and volunteers receive ongoing education and training.

Hospice House 5k & Remembrance Walk “Team Buddy Bear is made up of the family and friends of Barry “Buddy Bear” Greindl who passed away on March 29th, 2015. He was cared for at home for 10 months by his wife Gail, with help from Hospice nurses Tammy and Mary. During his final days, he was moved to the Hospice House in Auburn where he lived in comfort and peace for three days before his passing. It was his wish that he would simply fall asleep and not wake up again and that is exactly what happened. On August 9th, friends and families gathered once again at Androscoggin Home Care & Hospice’s Hospice House for the 6th Annual Butterfly Release Celebration. More than 500 butterflies were released in honor of loved ones lost, symbolizing life’s transformations. In addition to serving as a beautiful memorial event for our communities, the event raised more than $21,000 for patient care, volunteer support and bereavement services for patients and their families served at the Hospice House.

Autumn Night Out
The 2014 Autumn Night Out was held at the beautiful and historic Poland Spring Resort with Norm Karkos, of WMTW Channel 8, serving as MC for the evening. The night consisted of silent and live auctions, dinner, games such as the newly introduced “Wall of Wine” raffle and music by the Don Campbell Band. Guests of the 2014 Autumn Night Out also had the special opportunity to be the first to view the agency’s new video highlighting services, staff and testimonials – it was a truly moving moment of the evening. The event raised more than $70,000 for hospice patients in need.

“My husband was in Hospice for 17 months and he had volunteers twice a week to play cribbage with him. I was very grateful and wanted to give back and help others. What I have found is that I get more out of the visits than I give. Whether playing cribbage, getting someone’s medication, or just a phone call with someone makes me feel good and gives me a feeling of peace for having helped someone.”
